Preventive Health Services definition

Preventive Health Services mean those primary preventive medical Covered Services, including related laboratory services, for early detection of disease as specifically described in the Preventive Health Benefits section of this Evidence of Coverage.
Preventive Health Services means health education, health supervision including evaluation and follow-up, immunization and early disease detection.
Preventive Health Services mean those pri- ▇▇▇▇ preventive medical Covered Services, in- cluding related laboratory services, for early detec- tion of disease as specifically described in the Pre- ventive Health Benefits section of this EOC.

  • If you receive both Preventive Health Services and other Covered Services during the Physician office visit, you may have a Copayment or Coinsurance for the visit.

  • If you only receive Preventive Health Services during a Physician office visit, there is no Copayment or Coinsurance for the visit.

  • All other benefits and services not specifically listed as Covered in the Benefits Section shall be excluded, except for Clinical Preventive Health Services and except as required by state or federal law.
