Prescription Plan. The Township shall provide husband/wife prescription coverage as outlined below in Section B.
Prescription Plan. For Officers who retire January 1, 2017 through February 28, 2017, the Township shall provide prescription coverage as outlined in Article 13.3(B) for the Retired Officer and Spouse who are not on Medicare. For Officers who retire on or after March 1, 2017, the post-retirement prescription plan for a Retired Officer and Spouse who are not on Medicare will mirror the plan provided to active Officers, and shall be subject to the same premium contribution rate as apply to active Officers.
Prescription Plan. The City shall provide a Co-Pay Prescription Plan for the Chief and any dependents through the Prescription Corporation of America. The City may change the plan or provider to a plan or provider other than the current Prescription Corporation of America Plan and in such event, the co- pays shall become the following: Co-Payments for Prescription Drugs (including Specialty Pharmaceuticals) other than Prescription Drugs dispensed by a mail order pharmacy: A $10.00 co-payment is required for Generic Prescription Drugs. A $20.00 co-payment is required for Brand Name prescription drugs. Co-payments for Prescription Drugs Dispensed by Mail Order Pharmacy: A $10.00 co-payment is required for Generic Prescription Drugs. A $20.00 co-payment is required for Brand Name prescription drugs. All benefits under the Prescription Drug Program are subject to the terms of the Group Policy.

  • If a Covered Person has a problem or complaint regarding any aspect of the administration of benefits by UK Prescription Plan, the Member may contact the UK HR Benefits Office or Express Scripts Customer Service to discuss the matter.

  • Management Plan means a plan to manage the activities and protect the special value or values in an Antarctic Specially Protected Area or an Antarctic Specially Managed Area.

  • Conservation Plan means a document that outlines how a project site will be managed using best management practices to avoid potential negative environmental impacts.

  • Implementation Plan means the schedule included in the Statement of Work setting forth the sequence of events for the performance of Services under the Statement of Work, including the Milestones and Milestone Dates.

  • Nonprescription medication means a drug or medication authorized pursuant to federal or state law for general distribution and use without a prescription in the treatment of human disease, ailments, or injuries.

  • Prescription drug order means an original or new order from a practitioner for drugs, drug-related devices or treatment for a human or animal, including orders issued through collaborative care agreements. Lawful prescriptions result from a valid practitioner-patient relationship, are intended to address a legitimate medical need, and fall within the prescribing practitioner's scope of professional practice;

  • Treatment plan means a written agreement between the department and the parent or

  • Investment Plan means the document in terms of which the special purpose vehicle will invest in unlisted investments and all matters incidental thereto;

  • Pest Management Plan and “PMP” means the Recipient’s plan dated December 13, 2010, which: (i) addresses the concerns relating to the Project risks associated with potential increases in the use of pesticides for agricultural production, intensification and diversification and controlling disease vector populations arising from irrigation schemes; (ii) sets forth mitigation and monitoring measures to be taken during Project implementation and operation to eliminate adverse environmental and social impacts, offset them, or reduce them to acceptable levels; (iii) recommends institutional measures to strengthen national capacities to implement the mitigation and monitoring measures; and (iv) as part of the implementation arrangements, identifies national agencies and other partners that could play a vital role in the success of the Project.

  • Nonprescription drugs means nonnarcotic medicines or drugs which may be sold without a prescription and are prepackaged and labeled for use by the consumer in accordance with the requirements of the statutes and regulations of this state and the federal government;

  • Quality Assurance Plan or “QAP” shall have the meaning set forth in Clause 11.2;

  • Forest management plan means a written plan prepared and signed by a qualified forester that prescribes measures to optimize production, utilization, regeneration, and harvest of timber. The forest management plan shall include a schedule and timetables for the various silvicultural practices used on forestlands, which shall be a maximum of 20 years in length. A forest management plan shall include all of the following:

  • Transition Plan means a transition plan, acceptable to the LHIN that indicates how the needs of the HSP’s clients will be met following the termination of this Agreement and how the transition of the clients to new service providers will be effected in a timely manner; and

  • Supply Plan has the meaning set forth in the CAISO Tariff.

  • Basic health plan model plan means a health plan as required in RCW 70.47.060(2)(e).

  • Nutrient management plan means a plan developed or approved by the Department of Conservation and Recreation that requires proper storage, treatment and management of poultry waste, including dry litter, and limits accumulation of excess nutrients in soils and leaching or discharge of nutrients into state waters.

  • Service Plan means the Operating Plan and Budget appended as Schedules A and D2a of Schedule D;
