PPW Housing Support Services means the costs incurred to provide support services to PPW individuals with children under the age of six (6) in a transitional residential housing program designed exclusively for this population. Program of Assertive Community Treatment (PACT)
PPW Housing Support Services means the costs incurred to provide support services to PPW Enrollees with children under the age of six (6) in a transitional residential housing program designed exclusively for this position. “Program of Assertive Community Treatment (PACT)” means a team-based, evidence- based mental health service delivery model that incorporates the values of Recovery and Resiliency. PACT is a package of already existing Medicaid State plan services provided by treatment teams, employed by a licensed BHA acting within the scope of its License. These teams also provide some non-Medicaid treatment activities, which are funded through GFS. PACT is client-centered, recovery-oriented, mental health service delivery model that utilizes a team approach providing services to Enrollees with severe and persistent mental illnesses and co-occurring disorders.
