POPULATION TO BE SERVED. A. In accordance with the Contract, Contractor is required, within the limits of the Contractor’s service capacity, to serve individuals who meet the financial and clinical eligibility criteria for an adult who is Seriously Mentally Ill (SMI) and is eligible for services as described in the DARMHA manual.

In accordance with the Contract, Contractor is required within the limits of the Contractor’s service capacity, to serve individuals who meet the financial and clinical eligibility criteria for an adult or child for Chronic Addiction (CA) and is eligible for services as described in the DARMHA Manual.
POPULATION TO BE SERVED. 15 CONTRACTOR shall provide services to children birth (0) to eighteen (18) years with 16 abuse or neglect issues who have been referred by Social Services Agency (SSA)/Children and
POPULATION TO BE SERVED. 11 CONTRACTOR shall provide services to children referred by SSA. Services will be 12 provided to Orange County children ages six (6) through eighteen (18) years, under court 13 jurisdiction, in out of home placements, including but not limited to children placed with resource 14 families or at Orangewood Children and Family Center (OCFC). The population to be served will 15 collectively be referred to as “YOUTH/YOUTHS.”
POPULATION TO BE SERVED. 1.1 CONTRACTOR shall provide services to Participants of Wraparound Orange County (Wrap OC) referred by ADMINISTRATOR. Participants include child/youth and Non-Minor Dependent (NMD) who meet any of the following criteria.
POPULATION TO BE SERVED. 18 1.1 CONTRACTOR shall provide services promoting safe and stable 19 families, to birth, kinship, blended, adoptive, and xxxxxx families with 20 children, ages birth through eighteen (0-18) years, who are at risk and/or 21 experiencing child abuse and/or neglect, families living in poverty or 22 economic hardships, domestic violence, unemployment, teen pregnancy, and 23 unhealthy parenting families receiving child welfare services, including 24 families in the family reunification and/or adoption process; homeless 25 families, unaccompanied homeless youth, and those at risk of homelessness; 26 non-minor dependents ages eighteen through twenty-one (18-21), who are being 27 served by child welfare or probation agencies and who are under the 28 jurisdiction of the Orange County Juvenile court; military families (active 1 and veteran); and persons with disabilities. The population to be served as 2 defined in this Paragraph shall hereinafter be referred to as “PARTICIPANTS” 3 or “FAMILIES."
POPULATION TO BE SERVED. 12 CONTRACTOR agrees to provide residential care and treatment services to Xxxxxx 13 Youth/NMDs referred to CONTRACTOR by COUNTY pursuant to the terms and conditions set 14 forth herein, in accordance with the Xxxxxx Youth’s/NMD’s Admission Agreement and
POPULATION TO BE SERVED. 11 CONTRACTOR shall provide Random Drug Testing Services by means of urinalysis to 12 all individuals, hereinafter referred to as “PARTICIPANT/PARTICIPANTS”, referred by the 13 Social Services Agency (SSA) Children and Family Services Division (CFS) of 14 ADMINISTRATOR. PARTICIPANTS to be served include parents and legal guardians of 15 children identified by ADMINISTRATOR to be at risk of abuse and/or neglect, dependent 16 minors/Non-Minor Dependents (NMDs) with a history of substance abuse whose case plan 17 includes required drug testing, PARTICIPANTS whose voluntary case plan includes drug testing, 18 or other PARTICIPANTS referred at the discretion of ADMINISTRATOR.
POPULATION TO BE SERVED. 15 CONTRACTOR shall provide Family Resource Center (FRC) services, as 16 contained in Paragraph 5 of this Exhibit, to: birth, kinship, blended, adoptive, and Resource 17 Families with children, ages birth to eighteen (0-18) years, who are at risk of or are experiencing 18 child abuse and neglect; families who are living in poverty or suffering economic hardship, 19 domestic violence, unemployment, teen pregnancy, and unhealthy parenting; families involved 20 with and/or receiving child welfare services; non-minor dependents ages eighteen (18) to twenty- 21 one (21) years old, who are being served by child welfare or probation agencies and who are under 22 the jurisdiction of the Orange County Juvenile Court; homeless families, unaccompanied homeless 23 youth, and those families at-risk of homelessness; military families; and persons with disabilities.
