Physician/Doctor definition

Physician/Doctor means a licensed health care provider acting within the scope of his or her license and rendering care or treatment to a Covered Person
Physician/Doctor means a registered medical practitioner qualified and licensed to practise western medicine and who, in rendering such treatment, is practising within the scope of his licensing and training in the geographical area of practice, but excluding a Physician or Doctor who is the Insured Member himself.
Physician/Doctor means a licensed health care provider acting within the scope of his or her license and rendering care or treatment to a Covered Person that is appropriate for the conditions and locality. It will not include a Covered Person or a member of the Covered Person’s Immediate Family or household.

Examples of Physician/Doctor in a sentence

  • If the District’s chosen Physician/Doctor requires additional appointment(s) to determine the Employee’s fitness for duty, the District shall pay the costs of the appointment and reimburse mileage as in number 2 above.

  • If the Physician/Doctor determines the Employee is not fit for duty, the District will not reimburse the Employee for the use of sick leave or pay due to lost time from work.

  • If the Physician/Doctor determines the Employee is fit for duty and the Employee has no sick leave time remaining, the District will reimburse the Employee for the lost time at their regular rate of pay.

  • When an Employee’s absence from work begins by the District requiring proof of the Employee’s fitness for duty, the District will pay for the Physician/Doctor appointment.

  • If the Physician/Doctor chosen by the District is outside the community the Employee works, mileage expense shall be reimbursed at the GSA rate per mile at the time of the Physician/Doctor appointment.

  • When an Employee’s absence from work begins with a document from a Physician/Doctor stating the Employee is unable to work, the Employee must have a document from a Physician/Doctor stating the Employee is fit for duty and can return to work.
