Payment Form definition

Payment Form means the manner of payment as specified in Section 2.5.
Payment Form means the Elected Payment Form or, if no such form is elected or is permitted to be elected by the Participant, the Default Payment Form, in each case for the payment of a Plan Benefit.
Payment Form means the document, documents, or electronic media prescribed by the Plan Administrator pursuant to which a Participant shall elect the time of distribution of his Account balance, and, with respect to the Participant’s initial year of participation except as otherwise set forth in Section 8.1(b)(ii), the form of payment of his Account balance, as provided under Article 8.

  • Each invoice submitted to City for payment shall include the information required in the HRC Progress Payment Form and the HRC Payment Affidavit.

  • Failure to provide the HRC Progress Payment Form and the HRC Payment Affidavit with each invoice submitted by Contractor shall entitle City to withhold 20% of the amount of that invoice until the HRC Payment Form and the HRC Subcontractor Payment Affidavit are provided by Contractor.

  • If Contractor’s failure to provide HRC Progress Payment Form is not explained to the Controller’s satisfaction, the Controller will withhold 20% of the payment due pursuant to that invoice until HRC Progress Payment Form is provided.

Payment Form means the Elected Payment Form or, if no such form is elected by a Participant, the Default Payment Form.
Payment Form means the document or documents, voice response or electronic media prescribed by the Plan Administrator pursuant to which a Participant shall elect the time and form for distribution of his or her Account balance, as provided under Article 7.

Payment Form means the Online Form which the Client has filled out to purchase the Services set out in this Agreement, detailing the pricing and details of the services to be provided by Pairview, as well as any payment plan.
Payment Form means the form in which a Member's benefit under the Plan is paid.
Payment Form means one of the following forms of payment on Retirement elected in a Participant’s Election:
