Patient Identifiable Information definition

Patient Identifiable Information means information relating to the identity, medical condition or history of any patient or prospective patient of the Authority or any other Health Board and any and all data generated and/or derived therefrom;
Patient Identifiable Information means any information (whether or not key coded) that identifies, or could identify, the Donor and/or any individually identifiable, or potentially individually identifiable, health information of that Donor, including, without limitation, Protected Health Information (as defined in 45 CFR § 164.501) or Individually Identifiable Health Information (as defined in 42 USC § 1320(d)) and other information protected by data protection and/or privacy legislation in any and all applicable territories.

  • Student personally identifiable information or “student PII” means information that, alone or in combination, personally identifies an individual student or the student’s parent or family, and that is collected, maintained, generated, or inferred by the district, either directly or through a school service, or by a school service contract provider or school service on-demand provider.

  • Individually Identifiable Health Information means information that relates to the past, present, or future physical or mental health or condition of the individual, or that relates to the provision of health care in the past, present or future, and that is combined with or linked to any information that identifies the individual or with respect to which there is a reasonable basis to believe the information can be used to identify the individual.
