Parent Declaration Form definition

Parent Declaration Form means the designated HA form used by Players whose parent(s) change residence in situations where the Player continues to reside with the parent;

Examples of Parent Declaration Form in a sentence

  • The distribution of hours must be recorded on the Parent Declaration Form (PDF).

  • DAF is paid to the setting nominated by the parent on the Parent Declaration Form and is a fixed annual rate of £615 per eligible child.

  • The Parent will complete a Parent Declaration Form and Section 4 contains information on providers and attendance details.

  • Please refer to the data privacy guidance set out in Annex 1: Parent Declaration Form, Step 4.

  • To claim the funding, a copy of the completed Parent Declaration Form along with a copy of the child’s Disability Living Allowance award letter must be posted to the Early Years and Childcare Service.

  • If the child does return during the term and the place has been left open then a new Parent Declaration Form must be signed to claim their remaining Free Early Education Entitlement funding.

  • The Provider should use the Parent Declaration Form at Annex A, which asks the parent for the necessary information and consents.
