Ongoing Support Assessor definition

Ongoing Support Assessor means a person, organisation or Commonwealth agency, contracted by the Commonwealth to conduct an OSA.

  • If the Provider disputes a Current Assessment, the Provider must contact DHS Assessment Services or the relevant Ongoing Support Assessor who conducted the Participant's Current Assessment, in accordance with any Guidelines, to discuss the reasons why the Current Assessment is not appropriate.

  • Program Provider to provide assistance to Ongoing Support Assessor If reasonably requested by an Ongoing Support Assessor, the Provider must provide all necessary documentation and assistance to the Ongoing Support Assessor to assist the conduct of an OSA or Change of Circumstances Reassessment.

  • Where DHS Assessment Services or the Ongoing Support Assessor does not agree that the Participant's Current Assessment is inappropriate, the Provider must provide Programme Services to the Participant in accordance with that Current Assessment (but subject to clauses 102 [Change of Circumstances Reassessment during Period of Service] and 110 [Change of Circumstances Reassessment during Ongoing Support].

  • At the interview with the Ongoing Support Assessor, the Provider is required to provide documents outlining the level of Ongoing Support provided to the Participant that could be considered relevant to an Ongoing Support Assessment.

  • Note: Where a Participant or DES Provider disputes the recommendation of an OSA Report, the matter should be raised with the Ongoing Support Assessor as soon as possible – see the Ongoing Support Assessment Guidelines for more information.

  • If it is clear that the Participant is not contactable for an interview then the Ongoing Support Assessor should record details of failure to contact the Participant, in the Participant Report section of the OSA.

  • The assessment of: the Provider under clause 121.1; or the Ongoing Support Assessor under clause 121.2, is the Participant’s Current Assessment until any further Assessment is undertaken.
