Needs and Services Plan definition

Needs and Services Plan means a comprehensive, individualized, time limited, goal oriented plan, developed and implemented by CONTRACTOR identifying the specific needs of an individual Placed Child, including, but not limited to, those items specified in Title 22, Division 6, Chapter 5, Section 87068.2 and 87068.22, that delineates those Services necessary in order to meet the Placed Child's identified needs.
Needs and Services Plan means the documentation of the child/youth’s specific behavioral 33 goals (including transition goals) and mental health services provided by the STRTP in consultation with 34 the CFT as required by Section 87068.2 of Title 22 of the California Code of Regulations.
Needs and Services Plan means a written comprehensive plan, that identifies all needs and services for a resident.

  • Other community resources may be used, e.g., mentor programs, job placement services, and vocational training programs, as indicated in their Needs and Services Plan.

  • The Needs and Services Plan shall be developed in partnership with 12 the child/youth, all of the child’s/youth’s treatment providers including the 13 County Social Worker, within thirty (30) days of the child’s/youth’s placement 14 in the program.

  • The Needs and Services Plan shall be developed by 20 CONTRACTOR in partnership with all of the ▇▇▇▇▇▇ Youth's/NMD's treatment 21 providers, including CONTRACTOR's Social Worker and ▇▇▇▇▇▇ Youth's/NMD's 22 family as appropriate, within the first thirty (30) days of placement.

  • The Needs and Services Plan shall be completed within 6 thirty (30) days of the Youth’s/NMD’s placement in the program.
