Medical Questionnaire definition

Medical Questionnaire means, with respect to a Referenced Settlement Recipient, the questionnaire concerning medical information, duly completed by or for such Referenced Settlement Recipient and in such form as provided by the Purchaser.
Medical Questionnaire means a series of questions designed to indicate the existence of a medical condition.
Medical Questionnaire means, with respect to any Claimant, a duly completed questionnaire in substantially the form attached as Exhibit G, provided, however, that during the Selling Period, the Purchaser may from time to time modify such questionnaire as it may determine in its reasonable judgment and such modified questionnaire shall become effective immediately upon delivery to the Seller and the Originator.
