Examples of Justice-involved in a sentence
These services will be provided to any eligible Yolo County resident referred through an access point established by Yolo County and will include, but not be limited to the following populations/programs: CalWORKS; Child Welfare Services; Perinatal; Criminal Justice Involved (AB109), including PC1000; Indigent; and Co-Occurring severely mentally ill and substance using clients.
The CONTRACTOR’s Care Coordination program description shall reflect Care Coordination requirements specific to special and high needs populations described in this Section 4.4.9. For JUST Health TOC Members, the CONTRACTOR shall complete the Justice Involved TOC Assessment/Plan/HRA template that is in Section 5.12 of the Managed Care Policy Manual.
On a case by case basis, SFAPD may consider referring to the Program Justice Involved San Francisco residents, not under SFAPD supervision.
The following Eligibles will be mandatorily enrolled with a Dental Benefit Manager under the SoonerSelect Dental program: • Expansion Adults; • Parents and Caretaker Relatives; • Pregnant Women; • Deemed Newborns; • Former ▇▇▇▇▇▇ Children; • Juvenile Justice Involved; • ▇▇▇▇▇▇ Care; • Children Receiving Adoption Assistance; and • Children.
These services will be provided to any eligible Criminal Justice Involved (AB109) or CalWorks eligible Yolo County resident referred through an access point established by Yolo County, or referred by the Sheriff Department for direct placement.
In accordance with the Centers for Medicare & Medicaid Services Updated Guidance to Surveyors on Federal Requirements for Providing Services to Justice Involved Individuals (May 3, 2016), to maintain custody of a justice- involved individual, e.g., an individual currently in custody and held involuntarily by the Santa Barbara County Sheriff's Office ("SBSO") (hereinafter "Inmate"), SBSO personnel shall be physically present with an Inmate sent to the PHF at all times, when assigned, to supervise an inmate.

The CCRT team initiative is funded by a Rider 48 Mental Health Grant for Justice Involved Individuals (MHG-JI) Grant through the Health and Human Services Commission (HHSC) for fiscal years 2025 and 2026.
