IV Form definition

IV Form means the pharmaceutical product in vials containing 10 mg or 50 mg of Compound (expressed as base) referenced in (i) new drug application No. 20-388 approved by the FDA and (ii) new drug application N° DIN 02 09 1283 approved by the HC.

  • Opt-Out Form means the form through which Eligible Employees may note their decision to opt out of participation in the Program.
