The Montana Department of Public Health and Human Services (DPHHS) has requested that we seek your consent to share your/your child’s immunization data with the DPHHS Immunization Information System (IIS).
Pursuant to its public health authority under the Public Health Act, section 24-1-3, NMSA 1978 to prevent, suppress, and conduct surveillance of disease and under the Immunization Act, sections 24-5-7 to 15, NMSA 1978 to establish and maintain a statewide immunization registry the New Mexico Department of Health (NMDOH) operates and maintains the New Mexico Statewide Immunization Information System (NMSIIS).
Reported immunizations are held in a central database known as ASIIS (Arizona State Immunization Information System), which can be accessed by providers to obtain complete, accurate immunization records.
The CONTRACTOR shall participate in the New Mexico Department of Health’s (DOH) New Mexico State Immunization Information System to ensure the secure, electronic exchange of immunization records to support the elimination of vaccine preventable diseases.
Such approval will not be granted if the system is not able to integrate with the Massachusetts Immunization Information System (MIIS).
Vendor shall remain registered at all times during the Term of the Agreement, and any Renewal Term, with the state’s Immunization Information System (“IIS”), the Illinois Comprehensive Automated Immunization Registry Exchange (“I-CARE”) system Vendor will also report all vaccination data to the State of Illinois, as specified in the Scope of Services, through I-CARE.
Provider must report required immunization data to the Nebraska State Immunization Information System, administered by DHHS/Division of Public Health.

Contractor shall ensure that member-specific immunization information is periodically reported to an immunization registry(ies) established in the Contractor’s Service Area(s) as part of the Statewide Immunization Information System.
