Home Clinic definition

Home Clinic means the program where an individual is admitted and primarily treated as a program service recipient.
Home Clinic means the ME LOCATION at which a CURRENT MEMBER entered into a MEMBERSHIP AGREEMENT or, if the CURRENT MEMBER transferred his/her MEMBERSHIP to a different ME LOCATION, then the “HOME CLINIC” for that CURRENT MEMBER is the different ME LOCATION to which the CURRENT MEMBER transferred his/her MEMBERSHIP.
Home Clinic means the Clinic to which you applied your membership. Elite members do not require a Home Clinic.

Examples of Home Clinic in a sentence

  • If you are unhappy with the alternative Home Clinic options, the reasons for this will be evaluated by YPP management, and the Early Cancellation process my be started.

  • Schedules will be posted with employees given preference to their Home Clinic by seniority.

  • If there is a different Home Clinic you would prefer to move your membership to you may do so in these circumstances.

  • Your membership allows you to access your Home Clinic in accordance with the type of membership option that you have selected as set out above.

  • We may have to cancel your membership of your Home Clinic with immediate effect if your Home Clinic permanently ceases operation.

  • We reserve the right to assign the benefit of your membership agreement or the ownership of your Home Clinic to another operator, firm or company at any time with or without giving you notice, as operational demands require.
