Grievance Report Form definition

Grievance Report Form shall be that as represented in forms Appendix E
Grievance Report Form shall name the employee involved, shall state the facts giving rise to the grievance, shall identify by appropriate reference all the provisions of this Agreement alleged to be violated, shall state the contention of the employee and of the Association with respect to these provisions, and shall indicate the specific relief requested.

Examples of Grievance Report Form in a sentence

  • In all levels of the formal proceedings the official Grievance Report Form shall be completed in triplicate: one (1) copy for the aggrieved, one (1) copy for the Association, and one (1) copy for the individual.

  • If the grievant is not satisfied with the disposition of the grievance in Step One, the grievant shall complete the Grievance Report Form, Step Two, and submit same to the Superintendent within ten (10) days of the receipt of its disposition at Step One.

  • If the grievance is not resolved by the informal procedure, or if the grievant elects not to use the informal procedure, it may be pursued further by submitting a completed Grievance Report Form (See Appendix).

  • If the grievant is not satisfied with the disposition of the grievance at Step Two-A or Step Two-B, the grievant, with the concurrence of the Association, may, within ten (10) days from the receipt of the Step Two answer, request a hearing before an arbitrator by completing Grievance Report Form, found in Step Three.

  • In the event the Grievant is not satisfied with the disposition at the informal level, or no decision has been rendered within ten (10) days after the informal meeting, it may be pursued further by filing a Grievance Report Form, Level One within ten (10) days after the informal decision was or should have been received.

  • The disposition by the individual(s) shall be added to the Grievance Report Form in triplicate within five (5) days of the hearing.

  • If the aggrieved member is not satisfied by the disposition of the immediate administrator, he/she may seek a hearing with the Superintendent or his/her designated representative within two (2) days after the disposition in Level Two, by completing Level Two on the Grievance Report Form in triplicate and submitting it to the Superintendent.
