The CONTRACTOR and its’ subcontractors employees shall obtain from the authorized Permit Issuer the necessary work permit forms required to perform the work being requested. It is the Permit Issuer’s responsibility to determine if gas testing is required to be performed and if so the types of gas test required performed and recorded on the appropriate work permit or permit forms. At the minimum whenever gas testing is required the Authorized Gas Tester must test for and record on the appropriate work permit or permits the % Oxygen (O2) and % LEL (Lower Explosive Limit). Additional testing that may also be determined required by the Permit Issuer include but are not limited to testing for Carbon Monoxide, Hydrogen Sulfide (H2S), benzene, and or total hydrocarbons,

The % Oxygen at the point being sampled must always be at or greater than 16% in order to ensure the accuracy of the % LEL reading. A “Hot Work Form” (permit) shall only be issued if the gas test results indicate 0% LEL. At no time shall hot work be allowed to be performed or the hot work allowed to be continued if the % Oxygen at the point being sampled falls below 16% or should the LEL be greater than 0%.
