The Mid-Term evaluation will be conducted after 16 weeks and the final evaluation will be conducted at the conclusion of the Field Placement. This evaluation will be conducted between the Field Instructor and the student. Once completed this will be reviewed with the Field Liaison to assess the student is meeting the objectives and tasks of the Learning Agreement. Use the following rating scale.
