Failed or "failure" means any of the following:

Failed indicates that a Transaction could not be processed due to inaccurate Information regarding your Account, insufficient funds or other error in the execution of the transaction.

Failed or "failure" means, consistent with rules adopted by the commissioner, a circumstance when a special purpose depository institution has not:
