Eye Examinations definition

Eye Examinations. Effective January 1, 2004, you can claim the cost of an eye examination to a maximum of $75 every two years.

  • Eye Examinations – a fifty dollar ($50.00) eye examination benefit which shall be available once in every twenty-four (24) consecutive month time period in accordance with the terms and conditions of the Plan.

  • Vision Care option (including Laser Eye Surgery and Eye Examinations) increased to $375 every 24 months.

  • The benefit levels are scheduled as follows: Eye Examinations - One regular eye examination every 24 months by an ophthalmologist or optometrist for each person covered under the program.

  • Eye Examinations for dependent children under age 21, 1 every 12 months and all others 1 every 24 months.

Premium Reduction); 11.6 (Savings Plan) and 23.1 (Eye Examinations), of the collective agreement during their probationary period.

  • Effective May 1st, 2007 – Eye Examinations to be included once every two (2) years.

  • The following vision care benefits will become effective January 1, 2016: Eye Examinations - One per year (12 month period) $150.00 maximum payment for frames, lenses, contact lenses, or any combination thereof.
