Extended Health definition

Extended Health. Blue Cross Drug plan, $2.00 per prescription drug card, no limit, private coverage if available, if not semi-private hospital. The $2.00 prescription drug card is effective from June 1, 1996. The para-med allowance for chiropractic services shall be $20.00/visit to a maximum of $500.00 / year.

Examples include, but are not limited to, a registered nurse (RN), nurse practitioner (NP), licensed practical nurse (LPN), medical doctor (MD), osteopathic physician (DO), respiratory therapist (RT), physical therapist (PT), physician assistant (PA), or occupational therapist (OT).

  • Behavioral therapy means interactive therapies derived from evidence-based research, including applied behavior analysis, which includes discrete trial training, pivotal response training, intensive intervention programs, and early intensive behavioral intervention.

  • Urban Coordinating Council Empowerment Neighborhood means a neighborhood given priority access to State resources through the New Jersey Redevelopment Authority.
