The Care Coordination and Disease Management Program Manager will direct all activities pertaining to Care Management and Care Coordination and monitor the utilization of Enrollees’ physical health and behavioral health treatments.
Liquidated Damages: 20% of annual fees for Disease Management Program Performance and activity reports will be submitted monthly to the Board.
