Dental Plan(s) definition

Dental Plan(s) means various plans for Payors outlining terms of coverage as provided by LIBERTY.
Dental Plan(s) means a dental program organized by a Payor which uses the Ameritas PPO network of Dentists who have agreed to provide Covered Services for which benefits or services are provided with an insurance policy or self-funded benefit plan.
Dental Plan(s) means the applicable dental benefits plan outlining terms of coverage provided by LIBERTY, in which plan Dental Office and its dentists are eligible, and selected and approved by LIBERTY, to participate. "Emergency" means procedures to evaluate and stabilize dental conditions of recent onset and severity accompanied by excessive bleeding, severe pain, or acute infections that would lead a prudent layperson possessing an average knowledge of dentistry to believe that immediate care is needed.

Related to Dental Plan(s)

  • Dental plan means any dental insurance policy, including those of nonprofit health service plans, and those of commercial group, blanket, and individual policies, any subscriber contracts issued by Health Maintenance Organizations (HMOs), and any other established programs under which the insured may make a claim. The term Dental Plan includes coverage under a governmental plan, or coverage required to be provided by law. This does not include a State plan under Medicaid (Title XIX, Grants to States for Medical Assistance Programs, of the United States Social Security Act, as amended from time to time.)

  • Retirement Plans means the retirement income, supplemental executive retirement, excess benefits and retiree medical, life and similar benefit plans providing retirement perquisites, benefits and service credit for benefits at least as great in value in the aggregate as are payable thereunder prior to a Change in Control.

  • Welfare Plans shall have the meaning set forth in Section 3.2.4.

  • Welfare Plan means a “welfare plan” as defined in Section 3(1) of ERISA.

  • Welfare Benefit Plan means each welfare benefit plan maintained or contributed to by the Company, including, but not limited to a plan that provides health (including medical and dental), life, accident or disability benefits or insurance, or similar coverage, in which Executive was participating at the time of the Change in Control.
