The information in this handbook is intended for Texas chiropractors, nurse practitioners (NP), clinical nurse specialists (CNS), certified nurse midwives (CNM), certified registered nurse anesthetists (CRNA), podiatrists, geneticists, maternity service clinics, physicians, and physician assistants.
For this period the Company will continue to facilitate the creation and transfer of subscriptions and provide CNM Credits to Subscribers in accordance with this Schedule and the CNM Rider.
For each Subscription, the Customer will receive a CNM Credit on their monthly bill from the Company.
For each Subscription, a Subscriber will be enrolled in the Company’s Community Net Metering rider and receive a monthly CNM Credit as set forth in the rider.
Information regarding history of any actions taken against any hospital privileges held by the CNM will be obtained.
