Claimant Information definition

Claimant Information means medical, health, financial and personal information about a potential Claimant or a Claimant or any spouse of such Person obtained in connection with a Claimant Purchase Agreement, including a Claimant’s name, street or mailing address, electronic mail address, telephone or other contact information, employer, social security or tax identification number, date of birth, driver’s license number, state identification card number, financial account, credit or debit card number, health insurance information, or photograph or documentation of identity or residency (whether independently disclosed or contained in a Settlement Package).
