Carers Assessment. Carers may have their needs assessed as part of the single shared assessment of the cared-for person or separately from this. Carers assessments are completed in line with SSA using the Carers Assessment tool and care plan. These forms have been devised in conjunction with carers and staff. Joint training for staff in carers right to assessment and guidelines for completing assessments have been provided in 2004 and will be repeated to extend the coverage. SSA and Care Management To supplement the core SSA tool when a more comprehensive or more detailed assessment is needed by care managers who are working with the most complex cases, an additional form will be introduced. This is currently out for consultation to teams across Grampian, with draft guidance. Supporting People and Housing A Supporting People Specialist – Housing Support Assessment can be used as a stand-alone assessment or as part of a single shared assessment. At present supporting people staff are not taking a lead assessor role in SSA but this is being addressed with a view to establishing the role of staff in SSA and offering training,. Supporting People staff are keen to see their role develop. Housing services are not involved in undertaking single shared assessment though they may be in contributing to assessments by health and social care staff and there are generally good working links between staff at a local level. The role of housing in SSA is being addressed through key staff in housing and the H&CC Development Group. Once this is decided the appropriate staff will be offered training and support at a local level for implementing arrangements for SSA in 2004.
