Ambulance Service License definition

Ambulance Service License means the legal document issued to an Ambulance Service by the County as evidence that the applicant meets the requirements for licensure to operate an Ambulance Service as defined by County Regulations.
Ambulance Service License means the documents issued by the Authority to the owner of an ambulance service when the service is found to be in compliance with ORS chapter 682; OAR chapter 333, division 255 and these rules.
Ambulance Service License means the authorization issued by the Board to operate an Ambulance Service, publicly or privately, within Summit County.

  • Issuance of a New Ambulance Service License: Upon receipt of a complete application for a license to provide Ambulance Services and payment of licensing and permit fees, the Board shall review the application, the applicant’s record and hold a public hearing to allow input from interested parties.

  • Each applicant for an air ambulance license shall pay the required fee as specified in Section 401.34(1)(j), F.S., and submit an application to the department on DH Form 1575, 08/15 04/09, Air Ambulance Service License Application which is incorporated by reference and available from the department, at www.floridahealth.gov as defined by subsection 64J-1.001(9), F.A.C., or at http://www.fl-ems.com.

  • Ambulance Service License Renewal: Application for renewal shall be filed annually, but not less than thirty (30) days before the date the license expires.

  • Issuance of a Renewal Ambulance Service License: The Board shall review the application, the applicant’s record and hold a public hearing to allow input from interested parties.

  • For any person, firm or organization except Public Safety Agencies in Ouachita Parish to respond to emergency or non-emergency ambulance calls that originate within Ouachita Parish, other than the Ambulance Service Provider that is the holder of a valid Ambulance Service License issued pursuant to this Ordinance or without the express authorization of the Operations Contractor.

  • Cancellation or material alteration of any required insurance policy or coverage shall result in the automatic revocation of any Ambulance Service License issued hereunder, and the Ambulance Service Provider shall thereupon cease and desist from further ambulance service operations in the Parish.

  • This Ambulance Service License authorizes the Provider to respond to Emergency and Non-emergency Ambulance Calls.

  • To use, or cause to be used, an ambulance service other than the Ambulance Service Provider holding a valid Ambulance Service License pursuant to this Ordinance.

  • Ambulance Service License – Every Ambulance Service Provider as defined in Section 6.7 responding to Emergency Ambulance Calls and Non-emergency Ambulance Calls originating in the Parish shall be required to obtain an Ambulance Service License pursuant to this Ordinance.

  • The permittee shall inform the county of any suspension and/or revocation of their California Highway Patrol Ambulance Service License, or Vehicle Certificate, or Authorized Emergency Vehicle Permit for any of their vehicle(s).

  • Ambulance service means an entity, its personnel and equipment including, but not limited to, automobiles, airplanes, boats or helicopters, which are licensed to provide Emergency medical and Ambulance services in the state in which the services are rendered.

  • Ambulance service provider means Latrobe Health Services recognises the following providers for the purposes of paying benefits:

  • Maintenance Services means SAP’s then-current maintenance and/or support services offered under and described in detail in the applicable SAP PartnerEdge Model.

  • Hospice services means palliative and supportive care and other services provided by an interdisciplinary team under the direction of an identifiable hospice administration to terminally ill hospice patients and their families to meet the physical, nutritional, emotional, social, spiritual, and special needs experienced during the final stages of illness, dying, and bereavement, as defined in Minnesota Statutes, § 144A.75, subd. 8, and includes the set of services as determined by the Medicare program under §1861(dd) of the Social Security Act and defined in 42 CFR § 418.3.

  • Software License means a license for the Software granted under this XXXX to the Licensee;

  • Online Services means Microsoft-hosted services to which Customer subscribes under this Agreement. It does not include software and services provided under separate license terms.

  • Directory Assistance Service includes, but is not limited to, making available to callers, upon request, information contained in the Directory Assistance Database. Directory Assistance Service includes, where available, the option to complete the call at the caller's direction. "Directory Listings" or "Listings" are any information identifying the listed names of subscribers of a Telecommunications Carrier or Provider, as applicable, and such subscribers' telephone numbers, addresses, or primary advertising classifications (as such classifications are assigned at the time of the establishment of such service), or any combination of such listed names, numbers, addresses or classifications: (1) that the Telecommunications Carrier and/or Provider provides or uses for the purpose of publishing the Listings in any directory format; or (2) that the Telecommunications Carrier and/or Provider provides or uses in Directory Assistance Service, Directory Assistance List Service, or for other lawful purposes.

  • License Key means a unique key-code that enables Licensee to run Software subject to the obtained User Pack.

  • Child support services means any civil, criminal or administrative action taken by the Division of

  • Retail Service and Maintenance Sector Means the service and maintenance of retail facilities which includes financial institutions, retail businesses, schools and hospitals but does not include commercial premises as defined.

  • Support Services the support services, as described in the Support Services Description, that We provide to You in respect of the Cloud Service.

  • Respite services means a temporary period of relief and support for individuals and their families provided in a variety of settings. The intent is to provide a safe environment with staff assistance for individuals who lack an adequate support system to address current issues related to a disability. Respite may be provided for a defined period of time; respite is either planned or provided in response to a crisis.

  • Application Services means the hosted applications and related services as described in Exhibit A.

  • Community support services means services authorized,

  • Service Territory means the geographic area within which PG&E as a Utility Distribution Company is authorized and required to provide electric transmission and distribution service.

  • Integrated Services Digital Network (ISDN means a switched network service that provides end-to-end digital connectivity for the simultaneous transmission of voice and data. Basic Rate Interface-ISDN (BRI-ISDN) provides for a digital transmission of two (2) 64 Kbps bearer channels and one (1) 16 Kbps data channel (2B+D).

  • Police Service ’ means the South African Police Service established in terms of section 5(1) of the South African Police Service Act;

  • Transportation Services means travel assistance given to an individual

  • Transportation Service Agreement means the contract between KUB and the Customer whereby KUB agrees to provide transportation gas service to the Customer.

  • Educational Services means educational training provided to Entitled Students at a School;

  • Hosting Services means the provision, administration, and maintenance of servers and related equipment, the provision of bandwidth at the hosting facility, and the operation of the Application for access by Customer Users to be provided by the relevant hosting service provider.

  • Program services means services that include all of the following provided they are pursuant to a program agreement: program needs assessment and development, job task analysis, curriculum development and revision, instruction, instructional materials and supplies, computer software and upgrades, instructional support, administrative and student services, related school to career training programs, skill or career interest assessment services and testing and contracted services.

  • Service lateral means an underground facility that is used to transmit, distribute, or furnish gas, electricity, communications, or water from a common source to an end-use customer. A service lateral is also an underground facility that is used in the removal of wastewater from a customer’s premises.

  • End User License means any license terms imposed by any Third Party Vendor on Customers and End Users. “Force Majeure Event” means an event or occurrence:

  • Technical Support Services means the technical support and maintenance Services provided by us according to our then-current technical support policy and procedure listed at xxxxxxxxxxxxx.xxx (“Technical Support Policy”) when the Services are purchased.

  • Company Services means any services (including but not limited to technical and product support, technical advice, underwriting and customer services) supplied by the Company or its Affiliates in the specialty property and/or casualty insurance business.
