Additional medical evidence means current physician examinations, clinical findings, laboratory findings, diagnosis, treatment prescribed with response and prognosis, hospital discharge summaries and diagnostic testing completed up to twelve months preceding the written notice of appeal that has not been previously submitted to the retirement system.
