This Agreement is made and entered into at the City of Muskego, Wisconsin, by and between the CITY OF MUSKEGO, a municipal corporation, hereinafter referred to as the "Employer" and the MUSKEGO POLICE ASSOCIATION, hereinafter referred to as the "Association."
AUTOMATIC PAYMENT PLAN AGREEMENTJuly 27th, 2017
Based on the above, I hereby authorize the City of Muskego Utilities to direct debit my account for my quarterly utility bill, which may include any/or all of the following: water, sewer, or refuse & recycling. The City of Muskego Utilities must receive authorization fifteen (15) days prior to the end of the quarter.
