This Amendment No. 2 ("Amendment") is effective as of the same date as that certain Master Services Agreement ("Services Agreement") e-signed by and between Mines & Associates, Inc. (MINES) and (hereinafter referred to as "Provider"), which Provider has an office in the Location of Practice, listed on the electronic signature page of the Services Agreement.
