This form details your agreement to abide by NCAA, Western Athletic Conference, Utah State University, and USU Athletics Department rules and regulations. The volunteer or part-time worker must obtain all signatures and meet with the Compliance Office prior to beginning any activities. Please also attach a detailed description of what your duties will be as a volunteer or part-time worker. Obtain the description from your supervising coach or unit supervisor.
