This agreement is between the Homeless and Housing Coalition of Kentucky, 306 West Main Street, Suite 207, Frankfort, Kentucky (“HHCK”) and the VISTA member (“Member”) ___________________________________ regarding the member’s service activities to be performed with an organization (“Sub- Site”) which has been selected by HHCK for placement of a member and which has a contractual relationship for the same with HHCK.
