This Teletherapy Master Professional Services Agreement (“Agreement”) is made and entered into as of September 25, 2023 (the “Effective Date”) until June 30, 2024 (the “End Date”), by and between AXIS Teletherapy, LLC, a New Jersey limited liability company (“Contractor”), and Big Spring School District, a K-12 public school district existing under Pennsylvania state laws (“Client”), for the provision of telehealth-based professional educational services. For the performance of this contract, the contractor will work closely with Jennifer Chamberlin, (contract manager), Supervisor of Ancillary Services.
