QUALIFIED HEALTH CENTER CO-APPLICANT AGREEMENT (the “Agreement”), by and between the North Broward Hospital District, a special taxing district of the State of Florida (the “District”), and Community Health Networks of Broward, Inc., a Florida not-for-profit corporation (“CHN”), takes effect upon the date of the last signature of the parties to this Amendment (“First Amendment Effective Date”).
