I, , being the parent or legal guardian of , a minor and an exhibitor (hereinafter referred to as Exhibitor) in the various 2018 Kentucky 4-H/FFA youth market livestock shows, am unable to be present during the validation process when Exhibitor’s animal(s) will be tagged with Kentucky Uniform Identification Program (K.U.I.P) tags and samples will be collected for DNA identification purposes.
