This Nonprofit/Charity Agreement is entered into and by LKSMART LLC, an Indiana, Limited Liability Company d/b/a RewardYourCharity.com, located at 1950 East Greyhound Pass, Suite 18-265, Carmel, Indiana 46033, who operates an online service RewardYourCharity.com website (“RYC”); and the undersigned nonprofit organization duly authorized and conducting business in the State of Indiana (hereinafter referred to as “NONPROFIT”). The parties of this Agreement will be referred to collectively as the “PARTIES”. The effective date of this Agreement shall be the date it is fully signed and executed by NONPROFIT indicated below (hereinafter referred to as the “EFFECTIVE DATE”).
