I/We have named the Unitarian Church of Evanston as a beneficiary of my/our estate or in my/our Will or Trust, and I/We wish this bequest be made payable to the Unitarian Church of Evanston Endowment Fund upon my/our death. It is my/our intention that this bequest shall be delivered to, held by, and administered in accordance with the terms of the Unitarian Church of Evanston Endowment Fund upon my/our death.
