For good and valuable consideration that I acknowledge as received, and with the intent to be legally bound, by signing this release I hereby irrevocably grant to , (“Producer”), his/her affiliates, licensees, agents and assigns (and their respective licensees and assigns) (“Assigns”), the unrestricted right to use the appearance, form, likeness and voice of the minor named below and contained in the photographs, film, or recording by Producer as part of a shoot described below (“Images”), and to distribute such Images for editorial, trade, advertising, packaging or other purposes in any manner or medium (including without limitation broadcast, online and print), whether now or hereafter devised, throughout the world, in perpetuity.
