Meal contracts are valid for the academic year and the office must be notified of any changes giving one week’s notice. Any changes not notified to the school office in writing will be charged to your child’s account. Any arrears may result in the withdrawal of school meal provision for your child.
FRESH EATS MEAL CONTRACTMeal Contract • January 5th, 2022
Each Meal will cost $7.00 per meal. A bill will be sent out monthly to you. Please make payments to Clarinda Regional Health Center on a timely basis. Please turn in this form with the driver who delivers your meal.
