In addition to the covenants, representations and warranties contained in the subscription agreement to which this Family, Friends and Business Associates Investor Package is attached, the undersigned Subscriber covenants, represents and warrants to the Issuer that the Subscriber is purchasing as principal and qualifies as follows under the prospectus exemption for “Family, friends and business associates” outside Ontario as required by sections 2.5 and 2.6.1, respectively, of NI 45-106 and has initialled or checked the appropriate line below and has signed a risk acknowledgement form.
