I, the undersigned applicant, agree to abide and enforce the rules, regulations, and policies governing this facility, as set forth by North Fayette Township. I understand that by signing this document, I accept all responsibility for any damages to premises, furniture, equipment, or grounds resulting from use of the facility. I further agree that any violation of the facility rules and regulations may result in immediate cancellation of the reservation and forfeiture of all fees and deposits. Non-township approved training/lessons/programs/practices are prohibited in all areas of the North Fayette Community Center.

RENTING THE GYM: Rental of the Gym will be handled through the Kenbridge Town Office during normal office hours. No activity shall run past 9:00 p.m. without prior approval. Gym will only be rented to persons 18 years or older. Lessee shall be present at all times during the rental period. Gym doors and exits shall remain unlocked and clear for the entire rental period. Adult supervision shall be provided whenever minors are present. If the Gym is rented for a fund raising event, or admission is charged, 10% of the proceeds will be paid to the Town of Kenbridge, in addition to the rental fee.
