This form should be filled in prior to each semester by students assigned a Graduate Research Associate (GRA) position or a Graduate Teaching Associate (GTA) position in the College of Public Health. The form must be submitted to the Graduate Studies Chair after being approved by the GRA/GTA supervisor.
