Use a separate form per sport and per field. By submitting this request, you assume responsibilities as the primary contact for the organization during the times of use. Please only request times you may need; failure to use field time as reserved may impact your standing status. The Town will dutifully work toward giving field time for any reasonable request. The Organization Contact Form and Field Use Agreement must be turned in with this request.
