This Member Services Agreement (this “Agreement”) is entered into by and between you, the undersigned patient and Strada DPC Program member, on behalf of yourself and, if applicable, your spouse and/or dependents (individually and collectively, “Member”) and the Strada-affiliated healthcare provider chosen by you (“Provider”). This Agreement shall be effective as of the date it is accepted and agreed to by Provider (the “Effective Date”). Now, therefore, in consideration of the mutual promises and covenants set forth below, the parties, intending to be legally bound, hereby agree as follows:
