WHEREAS, CITY and CONTRACTOR entered into an Agreement for Broker/Agent Services for Health Related Employee Benefits dated December 7, 2016, as well as a First Extension thereof dated November 6, 2019, hereinafter collectively referred to as the Agreement, by which CONTRACTOR provides Agent/Broker services regarding the evaluation and obtaining of insurance coverage and management of CITY’S self-funded group insurance program on and as-needed basis pursuant to RFP #16-63BK; and
