FAMILY PLANNING PROGRAM CONTRACT dated as set forth in the signature block below (this “Amendment”), amends that certain Affirm Sexual and Reproductive Health Family Planning Program Contract (the “Contract”) effective as of April 1, 2024, and entered into between the Arizona Family Health Partnership, dba Affirm Sexual and Reproductive Health, an Arizona not-for-profit corporation ( “Affirm”), and Pinal County Public Health Services District (the “Contractor”). Affirm or the Contractor may be referred to individually as a “Party” or collectively the “Parties”.
