This Agreement, including the attachments hereto, is made and entered into effective the 1st day of July 2014, between the Rhode Island Executive Office of Health and Human Services (referred to as “EOHHS”, or “Executive Office” in this Agreement) and UnitedHealthcare Insurance Company (the “Contractor”). This Agreement ("Agreement") is entered into in conformity with EOHHS procedures.
