This Vendor Services Agreement (“Agreement”) is entered into and is effective this day of , 2020 (“Effective Date”) by and between Remote Medicine, Inc., dba Remote Medical International (“RMI”), located at 4259 23rd Avenue W, Suite 200, Seattle, WA 98199 and the following “Vendor.” RMI and Vendor hereinafter are referred to individually as a “Party” or collectively as the “Parties.” Any capitalized terms are afforded the meaning of their definitions within this Agreement including any accompanying Schedules.
