These terms of service (the “General Terms”), to include all appendices attached hereto, including the Integrity Business Partners Application Form (“Application”), Merchant Services Agreement for Sub-Merchants (collectively, the “Agreement”) govern the Sub-Merchant’s participation in the Services. The Sub-Merchant agrees to be bound by the terms and conditions of the Agreement, as evidenced either by the signature of an authorized representative of the Sub-Merchant on the Integrity Business Partners’ Application or by the transmission of a Transaction receipt or other evidence of a Transaction. The parties to this Agreement shall be the Sub-Merchant as named in the Application that applies to participate in Transactions using Integrity Business Partners’ processing products (“Sub-Merchant,” “Merchant,” or “you”) and Integrity Business Partners, LLC with its principal place of business at 2355 Gold Meadow Way, Suite 215, Gold River, California 95670 (“Integrity Business Partners,” “we,
