This Class Action and PAGA Settlement Agreement (“Agreement”) is made by and between plaintiff Teri Flores (“Plaintiff”) and defendant Janesri De Silva, M.D. A Professional Corporation, Serendib Holdings Group, LLC, and Serendib Healthways, Inc. (“Serendib”). The Agreement refers to Plaintiff and Serendib collectively as “Parties,” or individually as “Party.”
